For water that is not delimited by coastlines you can draw both lakes
and islands as areas. Then tag the lakes as natural=water and the
islands natural=land, layer=1 and everything should be fine. The
direction of the ways should not matter.
Then you only run into problems with the maps rendered by t at h if a
complete z12 tile fits inside the lake.
