In the UK I can think of one motorway which is not entirely dual  
carriageway (the A601(M)) and Wikipedia claims that's true for part  
of the M58, too. Off the top of my head M50 j3 has two-way slip- 
roads, but then it was clearly designed by someone smoking crack anyway.

Which means that in the UK you'd be justified in tagging the minority  
with oneway=no, rather than the vast majority with oneway=yes. That  
of course requires the client to know about differing interpretations  
within national boundaries, but that's nothing new for OSM - roads  
already "inherit" speed limits from the country they're in, and so on.
