There is a "world" tab in the JOSM download dialog, and you can even  
zoom in there. However before you've reached a area that is small  
enough for a successful download, the display becomes pretty blurred.

That's where Tim Haussmann's slippy_map_chooser plugin comes in and  
does more or less exactly what you wanted:

"Adds a new download dialog that allows to select the download  
rectangle on the slippy map. With this plugin it's possible to  
download ways and gpx logs without knowing the coordinates of the  
desired download rectangle - just navigate to the desired area by  
zooming and moving the slippy map, mark the area with a rectangle and  
click download."
