2 in fact:

Moncrieffe Island is a small island in the River Tay at Perth [1]. It is
messed up, but I don't see any double nodes there.

Part of the River Clyde in Glasgow is missing [2], again, I can't see
anything wrong here, but the misrendering exactly corresponds to the
zoom 12 tile 1999 1278.
