I cant yet see the reason for [1]

but for [2]

the riverbanks are tagged as coastline to a point just  west of the George V 
Bridge  and the river continues with tags waterway = riverbank east of that 
point.

This means the coastline way is open . If you join the coastline way between 
the north and south riverbanks it should render OK I believe.
