cul-de-sac means 'dead end' or 'no exit', regardless of the presence
of a turning circle.
in answer to the original poster, why not include a size tag, which
accepts a diameter, in metres?
or, if it's a different shape to a normal (for your location...)
turning circle, use highway=residential, area=yes and draw the outline
small medium and large are so ambiguous and difficult to differentiate usefully
