Have a look at Lake Titicaca:

This has a large number of islands in it of a variety of sizes. However if you look at it in osmarender you will see there are a few rendering problems. This seems to be a bug in osmarender, as many other large lakes suffer from similar problems. At least, I've been unable to find a tagging scheme that overcomes them. Mapnik is fine though.
