I don't spend a lot of time mapping complex junctions, but here is my
two cents anyway:

What if you used name="Exit 91" and name="Exit 91A" for the two
motorway_link's? Are they really the "name" of those sections of
tarmac? I reckon it may well be, for all intents and purposes... You
could, on the other hand, argue that an "exit" conceptually can only
apply to a single node and not a way segment - but I'm not so sure.
Just another option to consider, if you haven't already. Cheers.
