[Tile-serving] [osm2pgsql] Conversion to C++ (#187)

I'd like to try to merge it soonish, as it looks like an exiting and really useful work to make it easier to work with in the future. But I haven't started looking at the PR in any detail or do any code review.

So probably the next step is for me to try out this branch and start looking at things to see if there are any issues left or additional work needed.
