[Tile-serving] [osm2pgsql] Add cmake build scripts and appveyor.yml (#460)

How do you run a single `make check` test? With autotools it's `make check TESTS=test/test-output-pgsql-schema`.

libosm2pgsql.a shouldn't be installed with `make install`, it exists only for the tests. default.style should be installed. nodecachereader is currently installed, but I'd be okay dropping that from the install if there are solid reasons. @woodpeck found it had been broken for over a year with 64-bit IDs, so clearly no one is using it.
