[Tile-serving] [osm2pgsql] Add cmake build scripts and appveyor.yml (#460)

Rebased to latest. The tests on Appveyor now pass, so the pre-merge setup of appveyor in the main repository can already be done (tick 'ignore branches without appveor.yml' in settings).
