[Tile-serving] [osm2pgsql] Add cmake build scripts and appveyor.yml (#460)

My custom-built boost thread used rt because I specified ``runtime-link=dynamic`` when building boost. Not sure if it can be detected.

Am I right that linking to ${CMAKE_DL_LIBS} and rt is safe on Linux, even if it is actually not needed?
