[Tile-serving] [osm2pgsql] Add cmake build scripts and appveyor.yml (#460)

I also found that some of the boost libraries in CMakeCache.txt are still from /usr. That may cause a conflict and I doubt the CMake is to blame here :) (I better build full debug+release boost)
