Your use of osm2pgsql is too far out of ordinary use that we can help to debug this here. If you can reproduce the problem in a simple test case (start with empty db, load simple osm file, load simple osc file with a change that should trigger a tile expiry but doesn't) we might be able to help you. We need all the data and scripts used in that case so we can reproduce the error.
