Thanks for all your input. Sounds like we are in agreement that this is a good idea. I'll do a PR.

@mmd-osm There has been a simple (but expert-only) option `--middle-way-node-index-id-shift` since version 1.4.0. The documentation you link is for creating the bucket index on databases from earlier versions.
