I suggest looking at this: https://github.com/HSLdevcom/hsl-navigator
It is service of Helsinki Regional Transportation, based on
OpenTripPlanner, which uses OpenStreetMap as one of the data sources. It
can do route plan that combines pedestrian routes with wheelchair + public
transport.
It is an open source project described here:
