[Accessibility] Require permission to do a school project about Wheelmap

Hi,

I am an Occupational Therapy student from the City University of New York. For one of my class, Community capstone class, this semester, we need to identify and contact a community organization and apply the occupational therapy practice theory complete a project for the class. We want to think outside of the box and look for organizations that have boarder impact in the community and can reach out to more people. We're very interested in this topic because we know the hardship that wheelchair users face when trying to access their community. However, we need you to complete a permission form (attached with the Email) in order to proceed with the project.
The project and the form will only be used for school work purpose and will not be used for profit. It will be a great honor to complete the graduate capstone project about Wheelmap. Please feel free to contact me for further information.
