The address data will need to be formatted for OSM. So for example, 1055
PARKFORD DR should be stored as

addr:housenumber=1055
addr:street=Parkford Drive

And so on. There's only 13 TYPEs, 4 DIR_PREFIX and 2 DIR_SUFFIX (along with
the above address, where "DR" is in DIR_SUFFIX instead of TYPE), so it
shouldn't be too bad even with a more or less manual process.

It also looks like much of the data in the UNIT field should be imported as
addr:unit. For instance, there are many residences at 1251 E LUGONIA AVE.
This impacts about 25% of the addresses.

You should also provide the data as it will be imported well ahead of the
import, so that it is clear things like the above are handled.

The source address data looks like it is pretty well maintained, so should
make for a smooth import.
