[Tagging] cave_entrance. ref and name

It seems not very consistent that the wiki suggests the tag "cave:ref" for
the ref of the cave but "name" for the name of the cave.

I would suggest "cave:name" for the name of the cave.
This way you can tag the name of the cave entrance as "name" and the ref of
it as "ref", while "cave:ref" and "cave:name" would be available for
tagging the name and ref of the cave to which the entrance leads.
Despite having been to a number of caves, I am not very familiar with how
cave cadastres work (particularly, whether there are different ones of
them), but I would expect to exist more than one of them, so it might make
sense to have the name of the cadastre as well in the tag (not looking at
the legal situation, depending on the license it might be forbidden to
reproduce the content of a cave cadastre).
