If the grounds are wholly part of the doctor or dentist clinic, then the
amenity way should be one the grounds not on the building. Tagging only on
the building is wrong.

Yes, this is the correct way to map.

Yes, amenity is for the facility, not the building, that is where the
building=* tag comes in. If the facility extent is limited to the building
only (ie. there are no grounds) then it makes sense to map the amenity=*
tag on the building=* way, but if there are grounds then they should be
different ways with building=* inside the amenity=* way.
