[Tile-serving] [osm2pgsql] Crash when updating with more than one thread (#229)

Thanks for testing, I need to set up a separate system to narrow things down. I did get the "dupplicate" message consitently, however, I was updating 10 hours of (global) changes which is naturally more likely to trigger a timing/concurrent/access/whatever bug.
