I saw both tickets but didn't have time so far to investigate further. I
suspect that both are not simple stylesheet issues but have to be fixed
in the rendering code. Which most likely means fixing it twice. Once for
or/p and once for xslt.

As a starting point it might make sense to look at revision 14384 or 12998·
